It is a mandatory policy that a parent/guardian views the athletic risk video one time throughout a students-athlete's secondary school years at Scottsdale Christian Academy.  Viewing of this video is required before a student-athlete is permitted to participate in practice or competitions of an SCA athletic team. 

The video is aproximately 9 minutes long. The video will begin to play in a few seconds, however, it may start and stop. If this happens you can put the video on pause until it finishes loading.  Please be patient. Once the video has been played in its entirety, a form page will appear. Please fill out this form and click the submit button to document that you have watched the video.
